Conference Policies and Waivers

By registering for the Dysautonomia International's 12th Annual Conference, you agree to the following policies and terms on behalf of yourself and all other individuals you register:

COVIDPOLICY2

COVID-19 Policy

Testing & Symptoms

We encourage all guests to self-test for COVID-19 before departing home for the conference. If you test positive or if you have recently been around someone who has COVID-19, please do not attend in person. If you are feeling symptoms of possible COVID-19, such as a fever, cough, shortness of breath, or other symptoms that may not be part of your typical dysautonomia symptoms, whether you test positive or not, we ask that you do not enter the conference. We will provide registered guests who can't attend in person the opportunity to view the livestream and to access the recordings after the conference. Please contact conference@dysautonomiainternational.org if you have any questions. 

 

Masking

We strongly encourage all guests to wear KN95 or N95 masks when indoors during the conference.


Air-Filtration

We will be using several Corsi-Rosenthal boxes throughout the indoor conference spaces, intended to reduce the risk of airborne pathogens. Learn more about Corsi-Rosenthal boxes here.

 

Despite these preventive measures, we cannot guarantee a COVID-free conference, so we encourage all guests to be fully vaccinated, wear a mask, and exercise caution on your travels to and from the conference. Guests who feel uncomfortable attending in person can register for the livestream broadcast or access to the recordings.
 

REFUNDPOLICY2

Refund Policy

  • Prior to and including May 20, 2024, your registration is fully refundable less a $20 service charge per registered guest.

  • After May 20, 2024, no refunds will be issued.

  • Registrations are not transferrable.

 

If you have already paid for your registration and would like a refund prior to or including May 20th, please contact us at conference@dysautonomiainternational.org

PETPOLICY2

Pet Policy

Pets are not permitted to attend the Conference. The only animals permitted at the Conference are service animals trained to work or perform tasks needed to assist a person with a disability, as defined by the Americans with Disabilities Act (ADA). Service animals must be under control of handlers at all times. If a service animal is out of control and the handler does not take effective action to control the animal, staff may request that the animal be removed from the Conference space to protect the safety of our guests and other service animals attending the Conference.

Liability Waiver

By registering for and attending Dysautonomia International's 12th Annual Conference ("event"), I agree to abide by all health and safety guidelines and recommendations promulgated by federal, state, and local government agencies and officials, Dysautonomia International, and the event venue, Loews O'Hare Hotel in Rosemont, Illinois. I further agree that I will not attend the event in person if I develop new symptoms consistent with COVID-19 such as fever, cough, shortness of breath, or a sinus infection, I am knowingly exposed to the COVID-19 virus within 10 days prior to the event, or exhibit any symptoms of the COVID-19 virus 10 days prior to the event. Additionally, I understand that travel to and participation in events may increase the risk of exposure to communicable diseases. 

I HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE ANY AND ALL LIABILITY, CLAIMS, AND DEMANDS OF WHATEVER KIND OR NATURE AGAINST Dysautonomia International AND ITS AFFILIATED PARTNERS AND SPONSORS, INCLUDING IN EACH CASE, WITHOUT LIMITATION, THEIR DIRECTORS, OFFICERS, EMPLOYEES, VOLUNTEERS, AND AGENTS (THE “RELEASED PARTIES”), EITHER IN LAW OR IN EQUITY, TO THE FULLEST EXTENT PERMISSIBLE BY LAW, INCLUDING BUT NOT LIMITED TO DAMAGES OR LOSSES CAUSED BY THE NEGLIGENCE, FAULT OR CONDUCT OF ANY KIND ON THE PART OF THE RELEASED PARTIES, INCLUDING BUT NOT LIMITED TO DEATH, BODILY INJURY, ILLNESS, ECONOMIC LOSS OR OUT OF POCKET EXPENSES, OR LOSS OR DAMAGE TO PROPERTY, WHICH I, MY HEIRS, ASSIGNEES, NEXT OF KIN AND/OR LEGALLY APPOINTED OR DESIGNATED REPRESENTATIVES, MAY HAVE OR WHICH MAY HEREINAFTER ACCRUE ON MY BEHALF, WHICH ARISE OR MAY HEREAFTER ARISE FROM MY PARTICIPATION IN THE EVENT.

Photo Release Waiver

 

Dysautonomia International and its staff, volunteers and contractors will be recording educational sessions and taking other photographs and video during the Dysautonomia International 12th Annual Conference.

I hereby grant the Dysautonomia International permission to use my likeness in a photograph, video, or other digital media (“photo” or "photos") in any and all of its publications, including but not limited to web-based publications, without payment or other consideration.


I hereby hold harmless, release, and forever discharge the Dysautonomia International from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.


I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS.
